Registration Form--Kids

    This form gives us important information about your child.  Remember, this should be filled out in addition to your signing up for the class on the website. This form does not count as registration for the class. 

    Drop off/Pick up:
    When you see me open the front door, have your child come to the door. (On the first day, feel free to join your child to see the studio. Or any day, really.)  At the end of class, I will bring the students to the door and make sure they see your car before going outside.

    Policies
    ~ Programs are designed to broaden the child’s interests.  Parents are asked to refrain from signing up a child for activities in which she has no interest.  A negative attitude impacts everyone’s experience.
    ~Yellow Brick Road reserves the right to cancel a program at any time if the minimum number of students has not been met.  A full refund will be issued in the event of course cancellation.  
    ~ Classes cancelled due to inclement weather or teacher absence will be rescheduled.  
    ~ Please be prompt when picking up your child.  This affects the start time of the next class.  A late fee of $1.00 per minute will be charged for any pick-ups later than 5 minutes. No early drop-off either, please.
    ~ Students will be expected to adhere to the same behavior that is expected of them during a regular school day.  Yellow Brick Road reserves the right to dismiss, without a refund, any child whose behavior detracts from other students’ ability to participate in and enjoy the program.
    ~Please refrain from sending a child who has any contagious illness, has had a fever or vomited within 24 hours.  Please call or text 703 928 2291 to cancel as soon as you know.
